How does the Program work?
Once you have finalized the sale, enter your information on this form including your name, your residential address, your phone number, e-mail address, the store number where you work.
Choose the model and number sold then enter the serial numbers of each machine.
Please use a separate form for each different model.
You will receive a confirmation e-mail when your information is processed.
